Social Worker II Psych FT Days

Position Summary:
The Social Worker plays a crucial role in coordinating resources to help patients achieve optimal health and access to care. Responsibilities include assessing patients for transition needs, conducting psycho-social assessments, promoting timely discharge, and preventing readmissions. This role also involves compliance with regulatory standards, educating patients and caregivers, and leading patient care initiatives.

Onboarding Process: Candidates must complete a background check and health screening, including a drug test.

Qualifications:
Required: Master's of Social Work. Preferred: 2 years of acute hospital experience. Must be licensed to practice in the state of practice. Lift up to 25 lbs. Push/pull up to 25 lbs. Sit frequently. Stand, walk, reach, stoop, and bend moderately. Demonstrate manual dexterity, mobility, and auditory ability.
