Job Description:
Join our esteemed client, a prominent figure in community healthcare for over a century, in their search for a caring RN Case Manager to be part of their Hospice Care Team. This role gives you the chance to utilize your nursing background in palliative care, hospice, or infusion therapy to impact the lives of diverse populations in need.
- Associate degree or higher in a relevant field
- Minimum of 1 year of hands-on experience in Palliative Care, Hospice Care, or Infusion Therapy
- Valid American Heart Association Basic Life Support (AHA BLS) and Ohio Registered Nursing license
Ideal Qualifications:
- 1+ years of experience in Triage and/or Pediatrics
- Bilingual language skills
Both recent graduates and seasoned professionals are welcome to begin an enriching full-time journey on the east side of Cleveland, OH.
- Providing direct care services to patients and families in various settings
- Applying nursing process principles for data collection and care evaluation
- Collaborating effectively with healthcare providers, third parties, patients, and families
- Ensuring patients' comfort and safety needs are met
- Utilizing nursing expertise for therapeutic and preventive care
- Communicating vital information to healthcare team promptly
- Responding to critical situations following nursing protocols and standards
- Coordinating with third-party payers for necessary supplies
- Staying updated on regulatory, licensing, and accreditation agencies
Work Schedule:
The role involves a regular 7 days on, 7 days off schedule, working from 4:30 pm to midnight with on-call duties from midnight to 8:30 am, including weekend coverage from 8 am to 5 pm. Occasional overtime may be required.
