The Opioid Health Home is a comprehensive medication assisted treatment program that provides a team approach made up of passionate, motivated, and dedicated professionals with the common goal of providing our clients with top notch care during their recovery journey.

The team is made up of MD's, NP's, LCPC'S, LADC'S, CADC'S Patient Navigators (MHRT/C's) and Recovery Coaches With this team approach, our members are fully supported in all aspects of their recovery journey.


The Substance Use Counselor is responsible for assessment, diagnosing, recommendation of treatment needs, and ongoing therapy to those with substance use disorder.


They are also responsible for the following:

  • Upon intake, orienting members to services provided, complete a comprehensive assessment, and support identifying appropriate level of care.
  • Develop an individualized treatment plan that reflects a team approach to care, support with implementing treatment objectives, and monitoring progress.
  • Maintain effective communication with members and the OHH team.
  • Attend team meetings and trainings.
  • Attend group and individual supervision
  • Provide individual treatment and/or group treatment based on member's needs
  • Each contact with member(s) will be clearly documented in a timely manner for the client's clinical chart and maintained by the clinician on a weekly basis.
  • Team coordination as it relates to OHH Consult with other OHH and primary care professionals.
  • Evaluate member(s) needs and determine if referrals to other programs or facilities are needed.
  • Provide DEEP Evaluations and treatment.

Experience/Qualification
The counseling position requires appropriately educated staff as set forth by the Department of Substance Abuse, State of Maine.

The individual qualified must have a minimum licensure of LADC, LCPC-c, LCSW, LMSW-cc, and LMFT, or may be a CADC working towards licensure.

All disciplines must be educated in addictions with a minimum of 1-year experience. You are responsible to maintain any and all requirements to keep your licensure in good standing.

If at any time during employment the status of your clinical license or certification is revoked or placed on probation, you will be ineligible for continued employment in this capacity.
