The applicant shall manage patients in transition between levels of care, serves as an expert resource to implement and teach skills.

Qualification:
The applicant shall demonstrate evidence of education, training, and experience in Internal Medicine or Family Practice

The applicant shall have current, full, active, and unrestricted license and registration in the state.
The applicant must have a bachelor's degree in a health care occupation.
