Essential Job Responsibilities/Expectations
- Responsibilities include those in competency document.
- Maintain a professional image and exhibit excellent customer relations to patients, visitors, physicians, and co-workers.
- Provide administrative support to Case Management teams as needed.
- Answer multiple and designated telephone lines.
- Utilize computer skills at an intermediate level.
- Expand awareness of post-acute care settings.
- Coordinate patient transport services when required.
- Assist with front office tasks as necessary.
- Independently follow up with post-acute referral settings.
- Accurately document all care transactions in the medical record or system.
- Maintain payer fax line and communicate effectively.
- Perform other assigned duties.
Job Requirements
Qualifications/Training:
- Proficiency with Microsoft Word and Excel.
Licenses/Certifications/Registrations/Education:
- Minimum of a High School Diploma/GED from an accredited school.
