Job Summary:

Primary functions are to administer skilled nursing care under the supervision of a registered nurse, for patients/clients of all ages in their place of residence. Participate in the coordination of coordinate care with the interdisciplinary team, patients/clients and their families, and a referring agency.

Job Qualifications:

Education: Graduate of an accredited School of Vocational Nursing.

Licensure: Active state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver's license.

Experience: One year of experience as a licensed vocational/practical nurse in a clinical care setting required. Home health experience preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR.

Transportation: Reliable transportation and valid auto liability insurance.

Environmental and Working Conditions:

Works in patients'/clients' homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.

Physical and Mental Effort:

Prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients. Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition; meet patient/client and family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.

Essential Functions:

• Under the direction of the RN, assists in identifying the patient's/client's physical, psychosocial, and environmental needsas evidenced by documentation.

• Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.

• Provides patient/client and caregiver education.

• Provides care using infection control measures that protect both the staff and patient/client.

• Assures the continuity of care through delivery of quality patient/client care.

• Provides effective communication to patient/client /family team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.

• Monitor assigned cases to ensure compliance with requirements of third - party payor.

• Demonstrates commitment, professional growth, and competency.

• Promotes agency philosophy and administrative policies to ensure quality of care.

Salary: $ $33.00 per hour
