Job Title: Patient Care Advocate at Continuum Hospice

Continuum Hospice is looking for a motivated individual to join their team as a Patient Care Advocate. If you've got a background in hospice, a keen interest in venturing into marketing and sales, and are dedicated to our hospice mission, this could be the perfect opportunity for you.


As a Patient Care Advocate, you'll collaborate with current Liaisons to discover new referral sources, build lasting relationships, and enhance program census and preference for hospice services.


Responsibilities:Utilize your hospice expertise, empathetic communication skills, and marketing talents to engage referral sources, families, and potential patients.Act as a crucial resource to Liaisons and referral sources, ensuring exceptional patient and customer service.

Supporting Our Team:

Work closely with Liaisons, contribute to patient care by sharing clinical information, and engage in community events.


Building Brand Awareness:

Participate in brand awareness initiatives with Liaisons to establish our program as the preferred hospice provider.


Educating and Connecting:

Conduct meetings with patients and families, introduce them to hospice services, and make a meaningful impact in their lives.


Reporting and CRM Documentation:

Maintain regulatory compliance through meticulous record-keeping and timely submission of marketing reports.


Key Account Management:

Identify key accounts, provide insights to enhance our strategy, and support Liaisons and Regional Sales Director.


Requirements:

  • Bachelor's degree in related field preferred.
  • Minimum 1 year of hospice experience required, 2-3 years preferred.
  • Customer service or sales experience is a plus.
  • Valid driver's license and insured automobile in good working condition.
