Job Description
  • Review resident information and assess its impact on treatment
  • Conduct thorough functional assessments using standardized procedures
  • Develop individualized treatment plans with achievable goals
  • Document treatment plans accurately and maintain records
  • Provide supervision and instruction to Assistants
  • Inform and educate residents and families on treatment procedures and exercises
  • Collaborate with the interdisciplinary team to maximize resident's functional abilities
  • Stay updated on community resources for resident referrals
  • Participate in rehabilitation team conferences and quality improvement processes
  • Promote Heritage Healthcare programs and services
  • Adhere to professional standards and guidelines
  • Provide input on clinical performance and maintain productivity levels
  • Ensure compliance with regulatory requirements
  • Demonstrate knowledge of therapeutic principles and procedures
  • Support the company's commitment to Compassionate Care
  • Perform duties independently and assist others as needed


Qualifications
  • Bachelor/Masters of Science degree in Physical Therapy
  • Current state license
  • Supervision or experience in a relevant setting
  • Certification in specialized areas, if applicable
  • Strong communication and interpersonal skills
  • Competency in physical therapy methods and equipment
  • Ability to assess resident needs and create comprehensive care plans
  • Knowledge of accreditation standards
  • Physical ability to perform required tasks safely
