Medical Social Worker Hospice

We are currently seeking a Medical Social Worker with a Master's Degree for a PRN position at Star Hospice, a part of LHC Group. Responsibilities:
- Assess social and emotional factors related to patients' illness
- Assist patients and families in understanding and following medical recommendations
- Identify and utilize community resources
- Complete clinical notes and required hospice modules
- Provide bereavement services when necessary
- Collaborate with healthcare teams and community agencies
- Instruct on available community resources
- Mentor new social work hires and provide ongoing education
- Assist with policy development and revisions
- Qualifications Required:
- Master's Degree in Social Work
- 1 year of social work experience in a healthcare setting
- Current CPR certification and Driver's License

Additional State Specific Requirements for IL:
- Licensed as a Social Worker by the State Board of Social Work
