Responsibilities:


The Stroke and Heart Failure Program Coordinator, under the direction of the Telemetry Unit Director, serves as a Clinical and Program Coordinator for the Stroke and Advanced Heart Failure Program, ensuring quality, timeliness and effectiveness of patient care delivery.

Incumbent oversees both programs and maintains a strong component of patient advocacy with day-to-day review and coordination of multidisciplinary services.

This position is responsible for directing, evaluating, and providing feedback to professional level employees who participate in the operation of the Stroke and Heart Failure programs.

Incumbent assumes the day-to-day responsibilities of process and quality improvement activities as they relate to program performance in addition to developing, monitoring, and acting upon clinical and operational metrics designed to continuously improve service delivery.

Incumbent provides leadership for development, implementation and evaluation of clinical pathways, clinical procedures, guidelines and protocols.

Incumbent provides formal and informal staff education for professional, technical and ancillary personnel at Grand View Hospital and provides coordination, consultation and/or teaching in the areas of patient education and research for adult and geriatric patient populations, families and community members.

Incumbent serves as a liaison between the hospital and state and regulatory agencies.

This individual works collaboratively with the Stroke Program and Heart Failure Program Medical Directors in a mutually supportive relationship which is essential to the success of the program in achieving high quality, patient-focused outcomes.


Qualifications:

EDUCATION AND EXPERIENCE:
Bachelor's Degree in Nursing required
Minimum of two years clinical nursing experience in neuroscience and /or heart failure nursing required
Minimum of two years of providing community and professional education preferred
Over 6 months to 1 year Experience in curriculum development and principles of adult education preferred
Master's Degree MSN preferred

SKILLS AND ABILITIES:

  • Effective interpersonal skills, required
  • Demonstrated analytical skills, required
  • Excellent interpretation of complex statistical data, required
  • Possesses familiarity with Microsoft Office (Word, Excel, PowerPoint, etc.), required
  • Competency in the use of a variety of computer hardware and software systems., required

LICENSES AND CERTIFICATIONS:
Registered Nurse in PA Licensed, Upon hire, required
Basic Life Support (BLS), Upon hire, required
Certification by the American Association of Neuroscience , Within 2 years of hire, preferred
