Job Purpose:

This role involves providing hands-on support to patients and physicians, ensuring quality patient care through medication administration, pharmacological information, therapy monitoring, and adherence support.

Job Duties:

The Clinical Pharmacist plays a crucial role in enhancing pharmacotherapy care for Oncology patients, ensuring safe, effective, and cost-efficient drug therapy. Collaborating with the team, the pharmacist conducts disease management, medication reviews, and prior authorization assessments, focusing on patient outcomes and quality of life.

  • Medication therapy management, including reviewing and monitoring regimens, documenting interventions;
  • Evaluating treatment appropriateness based on clinical information;
  • Developing clinical criteria for medication reviews;
  • Ensuring compliance with regulatory and clinical standards;
  • Monitoring data and suggesting improvements;
  • Participating in process enhancements;
  • Updating knowledge and skills regularly;
  • Adhering to legal and ethical standards;
  • Performing additional assigned duties;

Experience/ Education:

  • Required Licensure/Certification: Doctor of Pharmacy (PharmD) and state Pharmacist license;
  • Years of Experience: Minimum 1 year as a clinical pharmacist with PGY1 & PGY2 oncology residency;
  • Skills: Analytical, good judgment, strong communication, pharmacology knowledge, technical competence, legal compliance, productivity, and attention to detail. Proficiency in Rx30 software is a plus;
  • Traits: Organized, honest, detail-oriented, reliable, quality-driven, empathetic, good communicator;
  • Other: Energetic, motivated, team player, confidential, respectful.
