Requirements

Job Responsibilities:
Assess patient conditions and collaborate with the healthcare team to develop and implement individualized care plans.

Administer medications, treatments, and therapies as ordered by physicians.

Monitor and document patient progress and respond to any changes in condition.

Assist patients with activities of daily living and provide emotional support.

Adhere to all facility policies, procedures, and regulatory requirements.

Collaborate with patients, families, and caregivers to provide education and support.

Maintain accurate and detailed medical records.

Job Requirements:
Valid Registered Nurse (RN) license in the state of practice.

Minimum of 2 years of experience in subacute care or a related field.

Current BLS certification.

Excellent communication and interpersonal skills.

Strong clinical assessment and critical thinking abilities.

Ability to work well in a team environment.

Attention to detail and strong organizational skills.
