The Transfer Center/Bed Control Registered Nurse (RN) 2 delivers patient-family centered care in a culturally competent manner utilizing evidence-based standards of quality, safety, and service while ensuring population-specific patient care is maintained.

Please note: The UHT Transfer Center/Bed Control RN 2 is trained in both roles/function, Transfer Center and Bed Control (in many institutions, two different departments), and can function interchangeably between the two at any given time. Mandatory minimum BSN degree completed upon hire and/or currently enrolled in a BSN program; all current staff have a BSN or higher.

Their detailed role/job description is as follows:

• Bed Control:Oversees Bed Placement and reviews all Admission orders from all areas of admission entry point throughout the whole UHealth system (i.e., ED, OR, IR, intra-facility transfers/admissions, Internal, Cath Lab, Direct Admissions, etc.)

  • Ensures appropriate unit/bed assignment of patients based on a detailed review of admitting order sets, medications/interventions/infection isolation ordered, and Level of Care (LOC) requested; match patient acuity and care needed to the Nursing skill set provided on each particular floor/unit.

  • Reviews transfer orders for LOC changes to expedite/ensure appropriate floor/unit assignments while communicating with the Interdisciplinary Team

    • i.e., if the stability of a patient on a Medical/Telemetry unit is questioned or a “Rapid Response/Code” is called, and the patient subsequently needs ICU LOC—Bed Control coordinates (in conjunction with the ICU Free Charge Nurse) the proper and timely triage of the patient to an ICU bed for higher level of care

  Reviews daily OR and Cath Lab schedule to ensure bed availability of known surgical admissions that will necessitate an inpatient/extended recovery stay post-operatively; coordinating with the Charge Nurses of surgical units evening prior/day of to ensure bed availability/appropriate placement of OR patients

  Coordination of add-on admissions/bed placement of patients coming from Interventional Radiology (IR)/other procedural areas that need extended recovery for observation, pain management and/or bleeding precautions (often a daily occurrence)

  Facilitates intra-facility transfers/admissions for those patients that require higher level of care/extended recovery pre-operatively or post-operatively coming from known UHealth intra-facilities i.e., Bascom Palmer, Sylvester, or Lennar

  Reviews all Direct Admissions provided on the “Daily Admission Log” (including UMHC (Sylvester) Direct Admissions) the afternoon prior to the expected date of admission to ensure appropriate bed is held for the upcoming admission

  • Ensuring all necessary protocol/requirements have been followed i.e., auth approved, admitting orders in, etc. If census allows, bed held by Bed Controller for known Direct Admission evening prior

  Opens and closes all overflow ED units in events of high critical census, ensuring all Interdisciplinary Team members are aware, including proper email communication sent regarding opening/closing of such unit and staffing/Leadership is adequately prepared; documents all historical opening and closing of units in Box.

  Coordinates room preparation and bed assignments for all HPPs in conjunction with EVS, Staffing, Nursing Leadership and Administration

  Escalation of special staffing requests i.e., 1:1 sitter, Baker Acted patients, etc. with Staffing Coordinators

  Communicates blocked rooms to Plant Ops/Telecom/Biomed, etc. to ensure rooms/issue are properly addressed

  Communicates all negative pressure room availability Q shift (nightly)

  Records all ED Admission >30 min delays (i.e., LOC incorrect, lack of Droplet Precaution orders) in Box and escalates to appropriate ED or Lab staff to facilitate assistance in the delay.

  Coordinates and communicates room blocks for hospital-wide improvement projects i.e., Remote Sitter Installation, Window-Sill projects

• Transfer Center:

  Coordinates with accepting Specialist on all Inpatient, Emergent (ED) and/or International Transfers to ensure throughput of patient care; The Transfer Center RN ensures collaborative efforts are maintained with members of the Interdisciplinary Team from the initiation of the transfer request ongoing until the patient arrives at a UHealth facility i.e., UHT, Sylvester or Bascom Palmer; providing internal and external customer service and maintaining a working knowledge of all hospital policies, procedures, and professional standards.

  Oversees all transfer requests from Referring Facilities seeking Inpatient (including Jackson Memorial Hospital Inpatient transfers to Bascom Palmer), Emergent (ED) and/or International Transfers.

  • Inpatient Transfers:

    • Once accepting Specialist is verified and Plan of Care/Length of Stay and Procedure/Treatment needed is determined, the UHT Transfer Center RN ensures patient is financially cleared for transfer—coordinating with UHT Admitting Department to ensure Insurance Authorization is approved and/or Medicare days are valid

    • If patient has full Medicare, Medicare availability is checked via SPOT portal system by the UHT Transfer Center RN

    • Ensures Reciprocal Agreement is obtained from the Referring Facility

    • Once patient is financially cleared, UHT Administrative approval is obtained from the AOC for the week or in more recent COVID times, the CEO/COO which can only approve transfers during the COVID pandemic and/or when UHT is at a critical census.

    • Coordinates with accepting Specialist and/or works with Hospital Medicine Triage Coordinator to assign a Hospitalist Team for Admission/Orders

    • Ensures Admission Orders are correct including LOC/Unit bed assignment

    • Ensures stability of patient to transfer including coordinating RN to RN handoff between Referring Facility RN and UHealth RN

    • Involves Case Management if a patient needs to be tracked for expedited transfer back to the Referring Facility in Reciprocal Agreement scenarios

  • Emergent (ED) Transfers:

    • Coordinates all ED transfers into UHT or Bascom Palmer ensuring MD to MD handoff is provided; if the transfer is accepted by a UHT /Bascom ED MD, appropriate RN to RN handoff is facilitated.

    • Maintains appropriate EMTALA protocol with each ED transfer

    • Ensures patient is stable for transfer including obtaining Vital Signs and stability to transfer from the referring MD and RN

    • Provides Transportation updates to appropriate ED

    • Alerts appropriate staff if a Helicopter transfer is needed including email and phone communication to Security, Plant Ops, Nursing Supervisor, and ED staff as well as provides updated ETAs from the Pilot/Ground Crew

  • International Transfers:

    • Works in conjunction with UHealth International to facilitate International transfers to UHT following all above steps for Inpatient transfers

      • UHealth International is responsible for ensuring financial clearance of the patient

    • Collaborates with the Air Ambulance company transporting the International patient to facilitate appropriate hand off to the receiving nursing unit and ensure patient stability to transfer

    • Ensures Case Management is involved for all International transfers to ensure International patient has means to be transported back
