Job Description
The Population Health RN Care Coordinator is a nurse care manager that functions within the multidisciplinary team as an advocate, educator and facilitator of resources for patients to ensure the appropriate access to evidence-based medical services that are patient-centered. The Population Health RN is: (1) an advocate: ensuring access to appropriate care and services, promoting self-care management and prevention; (2) a navigator: identifying and removing barriers to care and coordinating patient care services by utilizing the nursing process to help patients move through the healthcare continuum; (3) a collaborator: working within the multidisciplinary team to ensure a comprehensive plan of care in accordance with nationally recognized evidence-based standards; (4) an educator: engaging the patients and their caregivers in understanding and learning in the management of their diagnoses, treatment, and symptoms; (5) an expert: knowledgeable in care management principles and utilization of systems and community resources tailored to meet individual patient needs; and (6) support team based care in the physician practices. The RN Care Coordinator will manage a sub-population of patients who are enrolled in the Accountable Care Organization (ACO) or other value-based care programs in order to ensure the most effective and efficient use of health care resources to meet the patients’ needs. Goals include: optimizing patient outcomes in chronic care management, enhancing the patient experience, avoiding preventable inpatient admissions/readmissions, avoiding preventable/non-emergent ED visits, and increasing the delivery of recommended preventive services.
Minimum Education Required
Bachelor’s Degree in Nursing
Required Qualifications Registered Nurse with current Ohio licensure, current BCLS certification and Baccalaureate in Nursing required. Minimum 3 years’ experience in a physicians’ office or other primary health care setting preferred. Care Coordinator must function with a high degree of autonomy, communication and interpersonal skills. Must understand the health care continuum and have the ability to solve complex problems. Supervisory/preceptor experience highly desired.
