JOB SUMMARY

The Behavioral Health Pediatric Navigator is a non-treating provider who serves as a point of contact for pediatric patients and their families who present in the Pediatric Emergency room, and the pediatric unit. They will evaluate the patient and situation and provide resources and assistance with accessing clinical and supportive care services. The PBHN will help to remove personal, situational, and environmental obstacles to health care access and provide information related to activities that engage, educate and offer support to individuals, their family members, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up. They will be involved in the ongoing assessment of the program as well as outcomes management. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

Other duties may be assigned.

· Provide intensive initial contact for each patient, with a minimum of one post-intervention follow-up call, and more for those needing additional support.

· Develop and keep current a comprehensive listing of vetted area behavioral health providers.

· Develop a relationship with leadership at the schools, local mental health centers, pediatricians, clinical outcomes management staff, emergency department and pediatric staff.

· Provide a complete biopsychosocial intake assessment to referred families and coordinate subsequent referrals.

· Provide a flexible schedule in order to provide consultation for referring physicians, other healthcare providers, and families.

· Serve as an essential link between patients, families and all other care providers.

· Responsible for outreach efforts to establish and maintain positive working relationships with physicians, social services staff, and the broader health community.

· Encourage patients to have a stable and regular relationship with a primary care provider and their mental health provider/s.

· Assist in increasing access to high quality, culturally competent care through stable insurance coverage, transportation, and other needed social supports.

· Facilitate communication and follow-up with health care providers.

· Participate in the ongoing development and review of the program.

· Participate in the collection and analysis of outcomes data.

· Participate in site-based staff meetings and interdisciplinary project teams

· Responsible to comply with all pertinent regulations under HIPAA.

· Coordinate with partner agencies to provide evidence-based trainings to community mental health providers

· Provide education to patient, parents, caregivers using facts and evidence-based medicine, alternatives to treatment, medication effects and side effects.

· Documentation of navigation promoting continuum of care.

· Follow-up with Positive ED Suicide prevention Screening on triage form.

·Coordinate Backer Act Placement and follow-up with their family. Integrate the community mental health resources with other education.

· Provide education to RNs and providers as needed.

EDUCATION, EXPERIENCE AND QUALIFICATIONS

· Minimum of a master’s degree required, Social Work or related health field preferred.

·Licensed Clinical Social Worker preferred.

·Minimum of 2-5 years of experience of working within community behavioral health system.

·Level II background check required.

·Obtain certification in the appropriate specialty within one year of being hired.

·Knowledge of primary care practices preferred.

·Knowledge of community health resources and how to help clients take advantage of programs that will improve their overall health status.

·Knowledge of Florida laws and statutes as they relate to pediatric patients, psychiatric or addictions medicine patients or become knowledgeable through education and training within one year of being hired.

· Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.

·Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
