Registered Nurse – Case Manager Compensation Pay: $56.14/hr Mileage Reimbursement: $0.48/mile Registered Nurse – Case Manager Highlights Full-time, Monday–Friday, 8:00 AM – 5:00 PM On-call required one week per month Counties covered: Charlton, Camden, Glynn, Brantley Contract-to-hire opportunity Opportunity to make a direct impact on high-risk patient outcomes Registered Nurse – Case Manager Responsibilities Perform comprehensive assessments of high-risk members Collaborate with primary care providers to implement individualized care plans Continuously reassess appropriate levels of care and cost-efficient interventions Coordinate non-medical support such as housing and transportation to ensure treatment plan compliance Facilitate care transitions in collaboration with physicians, social workers, discharge planners, and claims professionals Engage specialty resources to achieve optimal outcomes Document clinical, functional, and fiscal outcomes accurately Identify opportunities for health promotion and illness prevention Act as a patient advocate, maintaining privacy and confidentiality Registered Nurse – Case Manager Requirements Associate’s or Bachelor’s Degree in Nursing or related field Current, unrestricted Registered Nurse (RN) license in Georgia (or compact) Case Management certification preferred Ability to analyze complex medical information and make informed decisions Preferred skills: wound care, catheter care, IV antibiotics, labs, drains, HCHB software If you think this Registered Nurse – Case Manager position is a good fit for you, please apply today
