Job Summary:
We are looking for a compassionate and dedicated Hospice RN Case Manager to service the North Dallas area (Plano, Lewisville, Richardson, etc.). In this role, you will provide comprehensive, patient-centered care for individuals with life-limiting illnesses. The RN Case Manager will coordinate all aspects of hospice care to ensure comfort, dignity, and quality of life for patients and their families, while adhering to hospice philosophy and organizational policies.

Key Responsibilities:

  • Patient Care Management:

    • Develop, implement, and regularly update individualized hospice care plans for patients based on their needs, goals, and preferences.
    • Provide skilled nursing care, including pain and symptom management, medication administration, and wound care, as appropriate.
    • Perform physical and psychosocial assessments to identify patient and family needs.
  • Care Coordination:

    • Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and home health aides, to deliver holistic hospice care.
    • Serve as the primary point of contact for patients and families, ensuring effective communication and care continuity.
    • Coordinate services such as durable medical equipment (DME), medications, and supplies.
  • Patient and Family Support:

    • Educate patients and families about the hospice philosophy, disease progression, symptom management, and what to expect during end-of-life care.
    • Provide emotional support to patients and families, helping them navigate the challenges of terminal illness.
  • Documentation and Compliance:

    • Maintain accurate, timely, and complete documentation of patient care and communication in accordance with regulatory and organizational standards.
    • Ensure compliance with hospice care regulations, including Medicare and state requirements.
  • On-Call Support:

    • Participate in on-call rotations to provide after-hours care and support as needed.
    • Respond promptly to urgent patient needs during on-call hours.

Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in Texas.
  • Minimum of 1 years of nursing experience, preferably in hospice, palliative care, or home health.
  • Strong clinical skills, particularly in pain and symptom management.
  • Knowledge of hospice care regulations, including Medicare Conditions of Participation (CoPs).
  • Certification in Hospice and Palliative Nursing (CHPN) is preferred.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficiency in electronic health record (EHR) systems.
  • Valid driver’s license and reliable transportation.

Working Conditions:

  • Work is performed in patients’ homes, assisted living facilities, or other residential settings.
  • Regular travel is required to visit patients and attend meetings.
  • May involve evening or weekend shifts as part of the on-call schedule.
