CHW agents will by provide information, tools, and encouragement to empower individuals and communities to improve overall health. CHW agent(s) will help bridge cultural knowledge and health literacy differences and improve communications involving community members and health agency. CHW agents will work with individuals, families, community members, and healthcare providers to identify strategies and solutions to increase healthy behaviors among underserved and underrepresented communities.The Community Health and Wellness agent will be responsible for teaching in person and virtual programs to families, individuals, and small groups. The CHW agent will sit on advisory boards, attend programs and events in targeted communities and schools as directed to promote and recruit individuals for programming.
