ID: 49596733 Shift: Days, M-F Description: MRH Rehab is a rural clinic that provides the unique opportunity to treat a variety of conditions, ranging from pediatrics to geriatrics, orthopedics, post-op, sports medicine, vestibular/concussion and long-term neurological conditions. MRH Rehab is patient and community centered and we just won 2022 Best of Moffat County and MRH Hospital was named 2022 Best Place to Work .ABOUT: We are a 25 bed, multi-specialty medical clinic and rehabilitation center located in beautiful, rural Craig, CO (just 45 miles west of Steamboat Springs, CO Ski Resort). Craig offers many opportunities for local outdoor recreation, camping, rafting, paddle boarding, kayaking, skiing/snowboarding, mountain biking, hunting, fishing, etc.. provide L4 ED care, inpatient care, diagnostic imaging, surgery, laboratory, special care unit, cardiopulmonary, oncology and chemotherapy services and more.SHIFT: Days, M-F, 40 hours guaranteed, No OvertimeCERTS: BLS, CO License or Compact License requiredEMR: EPICHOUSING: housing leads uploaded into our Medefis account under 'facility group documents'JOB DESCRIPTION: SLP working primarily with outpatient pediatrics, outpatient adults and some subacute inpatient coverage as needed & modified barium swallow studies.Position Purpose: To provide diagnostic and therapeutic services, patient/family education, and appropriate referrals to patients exhibiting a speech, language, or swallowing disorder. Maintain quality care for all patients through a coordinated, comprehensive, multidisciplinary approach; and providing assistance where communication barriers exist due to speech/language disabilities to enable each individual to reach his or her maximum level of independence.Essential Functions and Basic DutiesPosition Specific Performance Expectations: Coordinates and directs patient care services to patients of all ages to ensure that patient needs are met. Services include patient evaluation/ assessment, establishing and implementing a plan of care, assessing effectiveness of and modifying treatment to ensure goal achievement, and educating patients/caregivers. Interacts professionally with the patient and/or family/caregiver and involves them in the plan of care. Demonstrates the ability to apply therapeutic principles and practices based on current evidence and knowledge, appropriate clinical judgment, and patient preference. Adapts the delivery of services with consideration for patients' differences, values, preferences, and needs. Practices in a safe manner that minimizes the risk to patient, self, and others Produces quality documentation in a timely manner to support provision of care, according to current standards that are based on professional association and regulatory guidelines. Collects and analyzes data from selected outcome measures to monitor patient progress and the benefit of interventions. Communicates appropriately, professionally and clearly to physicians, staff, and hospital leadership. Submits appropriate & accurate documentation and timely billing/coding information for services rendered. Participates in ongoing professional development to maintain a level of clinical practice consistent with best practice standards. This includes seeking opportunities to gain new knowledge and skills and utilizing these newly acquired skills in the provision of care and for growth and development of the department and hospital. Critically assesses self to improve clinical and professional performance. Advocates for the speech therapy profession. Protects patients and employees by adhering to infection-control policies and protocols. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. Develops rehab and hospital staff by providing information; developing and conducting in-service training programs. Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. Strives to: promote health, create a safe and effective treatment environment, motivate others, have a good bedside manner, listen, analyze information, be quality focused. School-based speech language pathologists (in addition to above expectations): Collaborate with the school-based team to help determine and carryout Individualized Education Programs (IEPs); identify a student s annual goals and determine the services, supports, modifications, and accommodations that are required for the student to achieve them. Home health speech language pathologists (in addition to above expectations): Provide in-home speech therapy services to homebound patients and work in collaboration with the home health interdisciplinary team. Performs other duties as assigned. Maintains patient confidentiality at all times. Upholds regulatory requirements to ensure continual compliance with departmental, hospital, state and federal regulations and policies. Completes annual education, training, in-service, and licensure/certification requirements; attends departmental and organizational staff meetings or reads meeting minutes. Reports to work on time and as scheduled; completes work within designated timeframes. Establishes and maintains effective verbal and written communication and good working relationships with all patients, staff and vendors. Utilizes initiative; strives to maintain steady level of productivity; self-motivated; manages activity and time. Actively participates in departmental and facility performance improvement and continuous quality initiatives. QualificationsMinimum Requirements: Must be at least 16 years of age (21 for driving positions) Must be able to legally work in the United States Must be able to pass a background check Must be able to pass a drug screen and breath alcohol test (if applicable) Must complete employee health meeting Education/Licensure/Certification: Active Colorado Speech Language Pathologist License (DORA) or valid compact license Current CPR(BLS) certification (within 90 days of hire per MRH Policy) Valid Driver s License (Home Health only) Experience: Current Certificate of Clinical Competence granted by the American Speech Language Hearing Association, in Speech Language Pathology (CCC-SLP) preferred.
