Requirements:
- Required Skills: Conduct initial evaluation, Establish and revise goals, Assists patients with navigating the Department of Children and Families (DCF) process, Collaborative care, Coordinate services to home care and equipment following discharge, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to rehab and long term care facilities, Refers to other disciplines/resources, Advance Directives, Medicaid, Medicare, Prior Authorization, Adult protective services, Child protective services, Power of Attorney, Advance Directives, Case management, Admission Criteria, Appeals and Denials, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Prior Authorizations
Job Details:
- Pay: $2414.05/week - $2594.85/week
- Assignment Length: 15-week assignment
- Shift: 5, 8-Hour 08:30 - 17:00
- At least one year of experience required
