JOBPURPOSE:
The Care Manager isresponsible for care coordination, progression of care, andproactive discharge planning and is accountable for expediting thetimely and safe discharge for all patients in their caseload
Qualifications:
MINIMUMEDUCATION REQUIRED:
Graduate ofNursing Program
MINIMUM EXPERIENCEREQUIRED:
Two (2) years ofexperience in an acute or post-acutesetting
MINIMUMLICENSURE/CERTIFICATION REQUIRED BYLAW:
Current unrestricted registered nurse(RN) license, or eligible for licensure, in the state of Georgia,required.
ADDITIONAL QUALIFICATIONS: Certification in Caseor Care Management preferred. Experience using InterQual orMilliman preferred. Masters degree preferred
