Compensation:Salary range starting at $75k year, with potential for annualbonus. Compensation based onexperience*

Howyou will make animpact

  • Leadinga holistic, person-centered approach to assessment and service/careplanning
  • Conductingpre-residency assessments and screening of potential residents andmaking recommendations for an appropriate level ofresidency
  • Responsiblefor evaluating the psychosocial and activity of daily living (ADL)needs of residents and coordinating care and services asneeded. 
  • Workingwith the interdisciplinary team to communicate with residents andfamilies regarding changes in the plan of care and residents'condition
  • Providingindividualized psychosocial support to residents and their familiesvia family meetings or support groups and assists through thetransition process to a higher level ofcare
  • Partnering withIndependent Living and Post-Acute Social Workers to develop supportgroups and educational programming opportunities for residents andfamily members, as well as working together when transitioningbetween levels ofcare

Whatyou willneed 

  • Minimumof 1-year related experience and/or training in Long-termcare/assisted living preferred. Education will be considered inlieu ofexperience. 
  • Experiencewith the elderly and understanding of the dementia process and itsimpact on daily living and activityprogramming. 
  • Musthave knowledge of the geriatric population and the aging process,including the physical, psychological and social needs of theelderly.
  • Meetsstate-specific requirements for management certification ofassisted living and/or personal care facilities (within 90 days ofhire)
  • NationalCertification Council for Activity Professionals (ADCP or ADC) asrequired by theState
  • ACC (ActivityConsultant Certified), CDP (Certified Dementia Practitioner), orCTRS (Certified Therapeutic Recreation Specialist) ispreferred.
