Dynamic Work : Work within a vibrant team of healthcare professionals and gain valuable exposure to a variety of disciplines and patient needs. Holistic Approach : Conduct psychosocial assessments and collaborate on individualized care plans, making a direct difference in patients’ lives. Innovation : Engage in telephonic or virtual counseling to deliver evidence-based interventions and support patient wellness and self-management. Key Responsibilities: 60% – Conducting patient assessments, making referrals, and delivering interventions to ensure care needs are met. 20% – Documenting interactions and maintaining thorough, accurate records. 10% – Participating in meetings, trainings, and contributing to program development. 10% – Collaborating with internal and external agencies to improve patient care coordination. What You’ll Bring: Experience : 3–5 years of experience in a health-related field (hospital experience is a plus) Skills : Strong understanding of social services and community resources, with excellent organizational, communication, and interpersonal skills. Knowledge : Familiarity with Epic EMR preferred; proficiency in Microsoft Office and EMR documentation standards. Licensure : Active LSW license in New Jersey (in good standing).
