Job Summary

 

  • Responsible for the transportation of HCS Clients/Patients to and from appointments and/or other scheduled activities, as directed. Duties may include, but are not limited to: picking up the patient, assisting the patient as needed into the staff or patient's personal vehicle, driving patient to their scheduled appointment/activity, assisting patient based on patient need during appointment/activity, and then driving the patient back to their home following appointment/activity, while providing an excellent and safe experience for patient and staff. Personal Care Aide Driver will maintain safe and clean working environment complying with regulatory requirements, as well as HIPAA regulations. Demonstrates responsibility for professional communications with all parties, while representing CHAH and the patient. Completes work assignments within expected time frames.

Job Requirements

 

  • Education
      • Required & Provided by CHAH: Sixteen (16) hours of classroom training and four (4) hours of on-the-job training within thirty (30) days of first client contact.
      • Required: Five (5) hours of inservice training per year.
  • Experience
    • Required: One or more of the following experience:
      • Minimum six (6) months paid experience as agency homemaker, home health aide, or household worker.
      • Minimum one (1) year experience, paid or unpaid, in caring for children, sick or aged individuals.
      • Successful completion of a formal course in basic nursing arts or nurse aide training.
  • Skills
      • Ability to read, write, follow directions and perform duties with minimum supervision.
      • Ability to deal effectively and courteously with clients, families, and coworkers and exhibit an empathetic attitude toward the sick and elderly.
      • Provision of reliable transportation and a working telephone as required by nature of position.
  • Licensure/Certification/Registration
      • Required: Active Class E Driver’s License in the state they reside
