5. Work with unit-level leadershipto identify, monitor, and report trends that prohibit optimalpatient flow.

6. Acts as liaison with otherancillary departments to ensure identified and anticipateddischarge patients are triaged and streamlined accordingly. Checkson delays in all sub-processes, including lab, imaging, pharmacyconsults, dialysis and any other department influencing thethroughput of patients.

7. Works with casemanagement, social work and inpatient units to prioritize patientdischarges based off Geometric Mean Length of Stay (GMLOS) andsimilar indicators.

8. Using communication andproblem solving skills, sets priorities, triages own workflowprocesses, works as a team member, and delegates responsibilitiesto other team members as needed.

9. Leads,interacts, motivates and communicates with a wide range of staff,physicians, patients, family members and other hospital personnelin a professional manner.

10. May act as aresource internally managing the coordination and delivery ofeducation within the entity (for example, sepsis,etc).
