**Assignment Duration:** 6 month - 1 year
**Shift Details:** 7:30a - 5:30p M - F/weekly
**DETAILS:**
- Total # FTE’s needed: 1.0Setting:
- Outpatient Primary Care Clinic
- Duties: In-person patient visits, task box, med refills, etc.
- Required Procedures: none
- Patients per day/volume:18 - 20
- Responsible of Mid-Level Providers: No
- EMR: Epic
- Support Staff: MA/LPN
- Reason for Coverage: Provider on extended Medical leave + currently recruiting growth position
- Deal breakers: Need a locum who will do full time coverage or 2 weeks/mo min.
**SUBMISSION REQUIREMENTS:**
- Board Certified or Board Eligible
- Active WA License or IMLC (LOQ must be uploaded)
- Clean Malpractice/Background
- BLS, DEA
- Availability at time of name clear – REQUIRED: Specific dates and black out dates required for presentation
