POSITION SUMMARY

CLUES’ Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, will be providing immediate access to supportive, high-quality, culturally-responsive and integrated Behavioral Health services and pathways to wellness for Latinx Communities, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in Latinx communities.

CLUES’ Children’s Mental Health Case Manager provides case management services to children who meet eligibility criteria for a Severe Emotional Disturbance (SED), according to Minnesota statutes, who are residents of Ramsey County. They build professional relationships with children and families while helping them navigate the mental health and related systems. They develop client case plans, document contact with clients and other client service providers, and work with families to create a crisis plan. They participate in agency activities that enhance the capacity of CLUES to provide quality services.

Position Type: Regular
Schedule: Full-Time. Generally, Monday – Friday Office hours, with occasional evening hours to accommodate clients as needed. 
Estimated Pay: $21.40 to $25.00 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provide bilingual and culturally responsive case management services to children who meet eligibility criteria for a Severe Emotional Disturbance (SED), according to Minnesota statutes, who reside in Ramsey County.
  • Build professional relationships with children and families to assess for needed mental health-related services and help them navigate the mental health and other service systems.  
  • Provide regular client support and follow-up. Ensure compliance with county standard for number of required/logged hours of client contact and related support activities. Meet at least monthly with clients in accordance with agency policy.
  • Develop client case plans and crisis plans. Review and update case plans and crisis plans at least every 6 months.
  • Coordinate and facilitate access to needed community and county services. 
  • Maintain up-to-date client documentation, including case notes and agency required reporting. Ensure CASII assessments are completed for each client every 6 months.
  • Respond promptly to client crisis situations and seek support from supervisor in high-risk situations. 
  • Meet goals and outcomes set for case load and in-person client contact requirements (e.g., meeting at least monthly with each client, and serving 12 clients per month, etc.). 
  • Support clients at mental health related meetings, appointments and court appearances. Provide transportation as needed and deemed appropriate. 
  • Participate in individual and group supervision as required within the case management team and the department. Attend other meetings as required by contract or agency. 
  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards.
  • Cultivate working relationships with service collaborators and other appropriate stakeholders.
  • Promote the mission, operating values and agency services of CLUES within the community.
  • Carry out all other duties as assigned by supervisor.

  SUPERVISORY RELATIONSHIPS
  • This position has no supervisory responsibilities.

 

QUALIFICATION REQUIREMENTS Education and/or Experience  
  • Bachelor’s degree in Social Work, Psychology, or related behavioral science field required. Master’s degree in Social Work, Psychology, or other behavioral science field preferred.  
  • Per Minnesota state statute, candidates without a bachelor’s degree may only be considered if they satisfy one of the following:
    • Have 3 to 4 years of experience as a case manager associate;
    • Be a registered nurse and have a combination of specialized training in psychiatry and 3 years of work experience consisting of community interaction and involvement or community discharge planning in a mental health setting; or
    • Be qualified as a case manager under the 1998 Department of Human Services waiver provision and meet the continuing education, supervision and mentoring requirements.
  • Possesses a deep understanding of Latinx culture/values. The ideal candidate is rooted in the Latino community’s experience, culture, and traditions and can navigate mainstream systems and experience to make relevant cross-cultural connections.
  • Familiarity with local community resources and service systems that benefit children and families required.
  • Must be skilled in the process of identifying and assessing a wide range of client needs.
  • Experience in case management preferred. 
  • Must be comfortable discussing sensitive issues with clients and staff in an appropriate, effective manner.
  • Crisis de-escalation skills and experience preferred.  
  • Efficient and skilled in time management required. 
  • Bilingual in spoken English and Spanish; ability to read and write in English required and Spanish preferred.
  • Must have a deep understanding of Latinx cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients, community members; creating forms and documents; hiring and training staff; and in clinical supervision and consultation.
Language Skills 
  • Effective communication in English and Spanish, both verbally and in writing, required.
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.  
  • Ability to effectively present ideas and information in and organized and effective manner, including writing reports. 
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.  
Transportation 
  • Must have a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.
Computer Skills 
  • Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required.  
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties. 
  WORKING CONDITIONS Background check
  • Must pass a criminal background check, driving record check and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
