SOCIAL WORK CASE MANAGER
JOB SUMMARY
The social work case manager is responsible for developing and supporting service plans for kinship families (grandparents and other relatives raising children who are not their own). The case manager assesses needs and provides comprehensive support services and resource linkage to ensure that children have high levels of well-being, safety, stability and permanence. The case manager also facilitates support group meetings for children, teens and adults, provides parenting training (123 Magic and Parenting a Second Time Around), and assists with family outings, donation drives and other events related to these programs. This is an hourly, non-exempt position. Some weekend and evening hours required.
DUTIES AND RESPONSIBILITIES
- Completes home visits, care plans and annual assessments of family needs
Engage in monthly contact with client to discuss care plan goals
Maintain accurate documentation of service goals and outcomes
Monitor and evaluate each client’s progression though their case management plan and make revisions to the plan as needed
Facilitate and attend adult support groups including assisting with child/teen support groups as necessary
Conducts parent education and life skills training
Identify available community resources and make referrals for services
Performs timely data entry in Apricot and Wellsky
Participate in staff meetings, trainings and weekly case reviews with Program Director
Develop and maintain a professional and supportive relationship with staff
Foster a professional and welcoming environment in all interactions with clients, guest speakers, and colleagues regardless of differing identities, backgrounds, beliefs, and choices
Adheres to policies, regulations, and protocol regarding client confidentiality and information sharing, and maintenance and storage of client information, including but not limited to those by HIPAA, IRB, and ISDD
Other duties as assigned
KNOWLEDGE AND SKILLS
- Knowledge and basic skills in case management including documentation, resource identification and referrals
Excellent communication skills on the phone and in person
Cultural competence with minority client populations which have experienced racial and economic inequities, Adverse Childhood Experiences, mental health and trust issues, special needs (developmental disabilities) and/or substance abuse
Ability to plan, implement, evaluate and report on activities conducted
Ability to travel to home visits, support group meetings, and parenting classes
Ability to establish rapport with others as a requisite for healthy and therapeutic relationships
Knowledge of community organizations and social service programs
Ability to exercise initiative and problem solving
Experience and comfort working with children with and without special needs
Time management and organizational skills
- Group facilitation and training skills with all ages
- Proficiency with computers including Microsoft Office Suite, database programs, email, and electronic calendars
Bilingual English/Spanish Fluency a plus
MINIMUM QUALIFICATIONS
Bachelor’s or master’s degree (preferred) in social work or equivalent. Two years of experience in case management. Experience with families and children preferred. Certificate in Grandfamilies Leadership and certification in Standards of Quality for Family Strengthening and Support (training paid for by ISDD).
