Family Medicine opening in Louisville, KY_x000D_Full-Time & Permanent_x000D x000D_Seeking a BE/BC Family Medicine physician to join outpatient clinic in Kentucky._x000D x000D_- Internal Medicine, Family Medicine or Geriatricians_x000D_- Seeking physicians and clinicians who appreciate senior patients, excel in a collaborate environment and strive for positive outcomes for their patients. _x000D_- Small patient panel size: 400 patients (our physicians see patients an average of once per month)_x000D_- Monday Friday schedule, no nights or weekends_x000D_- Practice call telephonic only and only 2-3 week blocks per year_x000D_- Comprehensive benefits and retirement package_x000D_- Paid Time Off (PTO)_x000D_- CME allowance_x000D_- Unique, hands-on, three-week orientation program_x000D_- Blackbelt and leadership development training_x000D_- Partnership opportunity after one year of practice _x000D_- Primary care practice model assuming full risk and accountability for patient outcomes_x000D_- PCPs serve as the quarterbacks or general managers for total patient care _x000D_- Care team including dedicated nurses and cases management social workers that support physicians, so they can remain at the center of patient care_x000D_- Transitional care team that includes full scope of in-house consulting specialists_x000D_- On-site lab and prescription dispensary_x000D_- Patient concierge services including transportation to and from appointments, prescription delivery, holistic health services including acupuncture, yoga and tai chi and nutrition seminars._x000D x000D_Location:_x000D x000D_Louisville, Kentucky's largest city, sits on the Ohio River along the Indiana border. Every May, its race course Churchill Downs hosts the Kentucky Derby, a renowned horse race whose long history is explored at the Kentucky Derby Museum. Baseball is celebrated at the Louisville Slugger Museum and Factory, where Major League bats are produced and a giant baseball "slugger" marks the entrance.
