The Registered Nurse Transition to Practice Program (RNTTP) Residency Program Nurse Registered Nurse (RN) - Mental Health Residential Rehabilitation Treatment Programs (RRTP) functions directly to support the activities of the St. Cloud VA Health Care System.



Duties

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.

RN duties include but are not limited to:

  • Demonstrates the ability to provide nursing care based on utilization of the health history and physical assessment skills and provides assistance for less experienced staff.
  • Demonstrates knowledge of Code Green Policy and Procedure and provides leadership for less experienced staff.
  • Maintains appropriate intervention of 1:1 observation and provides leadership for staff
  • Conducts a comprehensive assessment of the patient's health and psychosocial needs, including health literacy status and deficits, and develops a plan collaboratively with the patient and family or caregiver.
  • Plans with patient, family or caregiver, the specialist, the PACT team, other health care providers, and the community to maximize health care responses, quality, and cost-effective outcomes.
  • Facilitates communication and coordination between members of the health care team, involving the patient in the decision-making process in order to minimize fragmentation in services.
  • Educate the patient, the family or caregiver, and members of the health care delivery team about treatment options (including but not limited to CVT, HT, evidenced based protocols), community resources, psychosocial concerns, and case management, so that timely and informed decisions can be made.
  • Empower the patient to problem-solve by exploring options of care and alternative plans to achieve desired outcomes.
    . Encourage the appropriate use of health care services and strives to improve quality of care and maintain cost effectiveness on a case-by-case basis.
  • Assist the patient in the safe transitioning of care to the next most appropriate level.
