Summary

The intermittent RN serves as a back up for the following Community Based out patient Clinic (CBOC), Moab UT, Craig CO, Montrose CO and Glenwood springs CO. He/she will be utilized to cover current full time employees planned leave or short term vacancies. If you like to travel and see new places this is the job for you.



Duties

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until April 30, 2024, or until position is filled. The first group of eligible applicants will be reviewed November 14, 2023. Subsequent reviews will be conducted biweekly until position is filled. You will be referred as vacancies become available.

Major Duties include, but are not limited to:

  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
  • Administers medications and procedures per established policies and guidelines.
  • Accountable for utilizing critical thinking skills and sound judgment to ensure positive outcomes, maximizing third-party collections and delivering quality patient care performance improvement activities.
  • Will develop and coordinate seamless transitioning of patients throughout the inpatient and outpatient care process with a focus on discharge planning needs. Demonstrates knowledge and ability to apply principles of evidence-based practice.
  • Collaborates with the Teamlet (patient, Primary Care Provider, and Clinical Support Technician) and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient).
  • Executes position responsibilities that demonstrate leadership, experience, and creativity in performing a variety of duties, including supporting the primary care providers in managing complex patient care in clinic.
  • Position potentially requires flexibility in schedule and assignments, as this is a 24/7 hospital.
