Clinical Coordinator

Do you have experience in working with individuals with developmental disabilities or mental health issues? Bring your clinical skills to a team-based workplace that puts people first. As the Clinical Coordinator, you will advance the company's mission of providing quality service by facilitating and providing case management and counseling services.

  • Provide case management and counseling services to an assigned individual caseload. Assess referred individuals and lead development, coordination, and evaluation of service plan in conjunction with treatment team, funding source, and key stakeholders.

  • Maintain current and complete records in accordance with corporate, contractual, and regulatory standards.

  • Provide ongoing monitoring and consultation to mentors regarding implementation of service plan, including pre-service introduction and continuing education.

  • For caseloads that include mentor individuals, make mentor and family home visits, and conduct mentor group meetings as required.

  • Assist with emergency coverage and crisis intervention support as needed, including participation in rotational on-call system.

  • Assist with legal, licensing, regulatory and certification activities as needed.

  • Participate in all activities designed to enhance and build an active network of mentors.

Qualifications:

  • Master's degree in human services; Bachelor's or Master's in Social Work if required by state.

  • Two years of related experience working with individuals with developmental disabilities or mental health issues.

  • Self-motivated and detail-oriented with ability to multi-task.

  • Demonstrated skills with motivating team and driving results to meet goals.

  • Valid driver's license in good standing with access to a registered vehicle with proof of insurance if providing transportation to individuals receiving services.

  • Appropriate licensing (LBSW, LMSW, LCSW or LCSW-C) if required by state and/or agency.
