**Job Description:** As a Nurse Practitioner in Internal Medicine at SSM Health St. Mary\'s Hospital in St. Louis, Missouri, you will play a key role in delivering high-quality primary care services to adult patients within an outpatient setting. You will collaborate with a multidisciplinary team to assess, diagnose, and manage a variety of acute and chronic medical conditions.
**Key Responsibilities:**
**Patient Assessment and Diagnosis:** Conduct thorough health assessments, including medical history reviews, physical examinations, and diagnostic tests to evaluate patients\' health status and formulate accurate diagnoses.
**Primary Care Services:** Provide comprehensive primary care services to adult patients, focusing on health promotion, disease prevention, and management of chronic illnesses such as diabetes, hypertension, cardiovascular diseases, and respiratory conditions.
**Treatment Planning and Management:** Develop and implement individualized treatment plans based on evidence-based practices, including prescribing medications, ordering and interpreting diagnostic tests, and coordinating referrals to specialists as needed.
**Patient Education and Counseling:** Educate patients and families about health conditions, treatment options, preventive care, and lifestyle modifications. Empower patients to actively participate in their healthcare decisions and self-management strategies.
