The Nurse (Care Manager) demonstrates the following:
Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning.
Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes.
Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities.
Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise.
Guiding, developing, and supporting staff from a leadership perspective.
Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
General description of assigned duties include but are not limited to:
Assessing the health status of individuals and groups of individuals.
Identifying health care problems that are amenable to nursing intervention.
Establishing goals to meet identified health care needs.
Planning a strategy of care.
Prescribing nursing interventions to implement the strategy of care.
Implementing the strategy of care.
Administering medications and treatments as prescribed by those health care providers authorized to prescribe medication.
Authorizing nursing interventions that may be performed by others.
Maintaining safe and effective nursing care rendered directly or indirectly.
Evaluating responses to interventions.
Teaching the theory and practice of nursing.
Managing the practice of nursing.
Collaborating with other health professionals in the management of health care.
