The Clinical Pharmacist Specialist - Inpatient (part time) position will be located within Pharmacy Service at the Ralph H. Johnson VA Medical Center. The pharmacist in this assignment will have the capacity and flexibility to practice in several clinical areas including: the Medicine Intensive Care Unit (MICU), Surgical Intensive Care Unit (SICU), Internal Medicine, Mental Health, Hematology and Oncology, and the Emergency Department (ED). The primary function of the clinical pharmacy specialist (CPS) is the provision of evidence-based pharmaceutical care services through direct and indirect interaction with patients and providers.
Major duties include, but are not limited to:
Providing clinical coverage in several wards/clinics as assigned..
Providing a higher level of clinical pharmacy practice by demonstrating advanced skills in medication management services and offering expert knowledge on pharmacology, therapeutics, drug-drug interactions, medication safety, compatibility guidelines and aseptic techniques.
Performs comprehensive assessment of patients upon admission to the hospital and as necessary during hospitalization for appropriateness of pharmacologic therapy. Designs, implements, monitors, and documents pharmaceutical care plans.
Actively engaged with providers and patients in the medication reconciliation process to promote patient safety and ensure adherence with regulatory & quality standards.
Ensures adherence with the VA formulary and P&T committee drug therapy & policy recommendations, including therapeutic interchanges and automatic substitutions procedures. Recommends alternative medications as needed and reviews all nonformulary or restricted-use medication requests for appropriateness.
Verifies computer prescriptions (with provider order entry) for patients in his/her assigned clinic area.
Participates with the pharmacy management in formulation and implementation of policies and procedures to assure accurate compounding and dispensing of all therapeutic and diagnostic agents and other supplies furnished by Pharmacy Service to beneficiaries through the various inpatient and outpatient functional areas.
Participates in the education and training of new employees or employees introduced to selected practice areas.
Assumes an ownership role for administrative activities related to advanced practice setting, including reviewing consults in a timely manner, accurately closing encounters, independently resolving provider and patient care issues, and assuring appropriate coverage for clinical services.
Responsible for keeping abreast of changes in the Pharmacy Service (i.e. formulary additions and deletions, interdepartmental memoranda, relevant station memoranda, and policies & procedures) as well as changes in clinical practice standards and guidelines.
Work Schedule:
Rotating schedule on a Part Time basis to include but not limited to - 7:00am-3:30pm, 7:30am-4:00pm, 8:00am - 4:30pm, 10:00am - 6:30pm, 11:00 a.m. - 7:30 p.m., 1:30 p.m. - 10:00 p.m., 2:30pm- 11:00pm, or 9:30am - 10:00pm (12 hours shifts for Emergency Department coverage only). Holidays, weekends and overnight rotation may be required.

Pay:
Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off:
37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave:
