Shift: 8:00 AM - 4:30 PM

Experience:

• 24 months of Home Health experience is required.

• 3 months of experience with Epic is required.

Requirements:

• Candidates must have a Wisconsin license or compact license in hand at the time of application.

• This role will require floating to multiple units

• Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

Additional Details:

For local submissions, please subtract 10% of BR to calculate Local Rate.

Monday-Friday

Top Qualifications: Prior home health care experience preferred.

Strong clinical experience and communication skills.

Mileage is reimbursed (visit to visit, office to visit and visit to office only. Workers will not be paid to and from home unless they are reporting directly to a visit from their home.)

Graduate of an accredited school of nursing required,

One year experience in med/surg and/or home care or hospice nursing preferred

Scrub Color/Dress Code: Scrubs any color, not provided

Weekends Required?: No

On Call Required?: No

1 reference (Manager/Supervisor reference type)

Driver’s license required

Certifications:
