Scope

• Provide music interventions for patients to accomplish individualized goals within a therapeutic relationship.

• Collaborates, educates and promotes music therapy within the Interdisciplinary team. Communicates regularly with the interdisciplinary team, completes necessary documentation and updates the RN Case Manager with any changes with patient and/or family.

• Works closely with patients and families to meet the individualized goals established.

Education and Experience Required

• Music therapy skills as defined as generally accepted therapy standards. Must be proficient at performing with at least one instrument.

• Ability to maintain effective working relationships with team members, patients, physicians, etc.

Key Responsibilities

• Patient Care - Demonstrate respect, sensitivity, and concern for patients needs with a professional and pleasant manner. Interacts cooperatively and with consideration towards patients, families and hospice staff. Develop and execute individualized treatment plans for assigned patients including specific therapy goals. Reevaluate and modify plans as necessary. Evaluate patient responses to music therapy through direct patient interview, monitoring changes in perceived stress level changes.

• Communication - Collaborate with the Hospice Interdisciplinary Teams to assess and implement a plan of care for patients and families utilizing music therapy. Confer with physicians, case managers, social workers, chaplains and facility staff as well as patient's family members to clarify patient needs and to discuss issues related to patient care. Communicate with the Hospice Interdisciplinary Team of the effectiveness of music therapy and any other necessary changes.

• Therapeutic Techniques and Facilitation - Include techniques using the knowledge of music therapy. Provide opportunities for individual, small group and large group participation, as appropriate. Develop the program to provide both regular scheduled activities and special events. Participate and assist in planning of the annual memorial services as appropriate.

• Other Duties as Assigned - Provides staff education on Music Therapy and its benefits. Assists with special projects as assigned by the Vice President of Hospice. The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
