Performs medical duties in hospital or outpatient clinic covering primary care and general family medicine, where patients may have interacting acute and chronic medical problems such as diabetes, hypertension, asthma, arthritis, and cancer.
Provides first contact, continuous, comprehensive primary care according to current standards of care, the patient centered medical home model or care, and relevant regulations.
Coordinates and/or integrates care across all elements of the healthcare system, including referral to specialty clinics/consults or for admission to the hospital when appropriate.
Participates in health care teams comprised of physicians, non-physician providers and ancillary staff, and in meetings to develop new, modify existing, and evaluate medical procedures with the objective of improving the quality of patient care.
