A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. This announcement is for an Inpatient Floater Clinical Pharmacist. The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. The primary functions will be the interpretation and verification of physician's orders, patient medication profile providing drug and policy information to patients and medical staff. Coverage areas include, but are not limited to, Inpatient Pharmacy, IV room, Infusion Clinic, Internal Medicine, Emergency Department, ICU, Mental Health.
Duties include, but not limited to:
Engages with providers, nurses and patients in the medication reconciliation process to promote patient safety and ensure adherence with regulatory and quality standards.
Reviews electronic medical record to monitor for appropriateness of drug therapy, including evaluation for adverse drug reactions, drug-drug and drug food interactions, disease due to iatrogenic cause, drug duplication, and unnecessary polypharmacy.
Performs patient interviews including medication histories and provides education and counseling, specifically addressing compliance, instructions on how to take medications, indications for use.
Incorporates pharmacoeconomic considerations into clinical practice to promote cost effective and evidenced-based therapy, considering but not limited to drug selection, dosage form, frequency of administration, duration of therapy, monitoring, and efficacy.
Communicates with providers to establish positive inter-professional relationships, influence clinical care decisions, and solve technical issues related to clinical practice independently as needed.
Ensures adherence with the VA formulary and P&T committee drug therapy & policy recommendations, including therapeutic interchanges and automatic substitutions procedures. Recommends alternative medications as needed and reviews all non-formulary or restricted-use medication requests for appropriateness. Provides patient and/or provider education where needed.
Identifies reports and appropriately manages medication-related complications, adverse drug reactions or events, and medication errors.
Verifies computer prescriptions or orders (with provider order entry) for patients in his/her assigned clinic area or other areas as assigned.
Reviews and enters orders for intravenous admixtures when necessary for large volume fluids, IV-piggybacks, admixtures, TPN's, narcotics and chemotherapy.
Reviews and interprets physician orders for IV and oral medications for accuracy eligibility, availability and therapeutic appropriateness.
Provide professional guidance on IV admixtures or related matters to medical and nursing staff. E. Directs pharmacy technician support, where applicable.
Precepts pharmacy students, PGY-1/PGY-2 pharmacy residents.
Participates in the education and training of new employees or employees introduced to selected practice areas.
Participates in the ongoing education of medical center staff as it relates to the provision of drug therapy and drug information as necessary.
Work Schedule
: Rotating schedule to include but not limited to, 7:00am-3:30pm, 7:30am-4:00pm, 8:00am - 4:30pm, 10:00am - 6:30pm,11:00 a.m.
