JOB REQUIREMENTS
Minimum Education
Holds an active Registered Nurse license and is eligible to practice in the state of Kentucky.
Current BLS certification.
Minimum experience
Two years' experience as a Registered Nurse.

ESSENTIAL FUNCTIONS
Performs patient assessments and direct nursing services as appropriate to specific age and ongoing assessments as necessary as well as home care needs.
Explains philosophy, purpose, scope and role of hospice to patients/families.
Prepares and implements treatment plan for patient and family, in collaboration with the patient and family and other members of the interdisciplinary team.
Orders needed medication, supplies, and equipment.
Instructs caregivers in the proper use of medication, supplies and equipment, and in the basic skills required to care for the patient in the home.
Functions as a liaison between patient/family and other care providers of the interdisciplinary team.
Takes call for Hospice as assigned by Hospice Coordinator.
Supervises Hospice Certified Nurse Aides.
Educates patient and/or caregiver on disease process and consults with physician as needed.
Reviews, educates, and updates medications accurately and timely.
