Position Overview: We are seeking a dedicated and experienced Physical Therapist for a Home Health position in Yakima, WA. The ideal candidate will have a strong background in physical therapy with a focus on home health care, bringing expertise from both inpatient and outpatient settings.

Responsibilities:
Conduct patient assessments and develop individualized treatment plans to improve mobility, strength, and overall physical function.
Provide direct physical therapy services to patients in their homes, adhering to established treatment protocols.
Educate patients and their families on exercise programs, therapeutic techniques, and safety measures to ensure optimal care and recovery.
Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to coordinate comprehensive patient care.
Monitor patient progress, document treatment sessions, and adjust care plans as necessary to achieve desired outcomes.
Utilize OASIS documentation and ensure compliance with home health care standards and regulations.
Maintain up-to-date patient records and submit timely reports in accordance with regulatory and organizational guidelines.

Requirements:
Minimum 2.5 to 3 years of experience as a Physical Therapist, with a strong preference for home health experience.
Graduated from an accredited school of Physical Therapy.
Valid WA State PT License.
Basic Life Support (BLS) certification from the American Heart Association (AHA).
OASIS certification preferred but not required.
COVID-19 vaccination card.

Skills and Experience:
Proficiency in conducting patient assessments and developing effective treatment plans.
Experience in home health care, inpatient, and outpatient settings.
Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
Ability to work independently and manage a diverse patient caseload.
Knowledge of home health care regulations and best practices.

Additional Information: This is a full-time position with a 5x8s, Day shift schedule, offering a competitive pay range of $50 to $55 per hour.
