OUR PURPOSE

Through proactive outreach and education, we aim to deliver the right care at the right time, empowering referral partners and prospective patients to navigate the HCBS healthcare journey with confidence and peace of mind.

Schedule: Full-time | 80 hours per pay period | Monday-Friday | 8 am – 4:30 pm

Scope:
Deliver white-glove customer service and support within assigned facilities to ensure a high-level of satisfaction and timely transitions of care. Collaborate closely with key contacts within strategically assigned facilities to ensure understanding of Hospice and Home Health services and benefits to achieve organizational referral, admission, and census goals and increase utilization of services within assigned accounts.  Engage in pre-discharge facility calls/visits to prospective patients at the assigned facilities' request upon receipt of the referral to determine appropriateness and deliver seamless care transitions for home health care and hospice business lines. Develop and execute annual education plan in assigned facilities and with key stakeholders in partnership with marketing, sales, & clinical teams. Manage all aspects of the organization’s care transition program & discharge follow-up calls, as assigned. Partner closely with clinical staff to ensure hospice readiness when appropriate.
Key Responsibilities:
Collaborate closely with key contacts within strategically assigned facilities to ensure understanding of services and benefits to achieve organizational referral, admission, and census goals and increase utilization of services within assigned accounts.  Create and execute sales plans to achieve goals. Document all activities in the organization’s Customer Relationship Management tool Engage in pre-discharge facility calls/visits to prospective patients at the assigned facilities' request upon receipt of the referral to determine appropriateness and deliver seamless care transitions for home health care and hospice business lines. Rounding with other key healthcare staff to identify needs for services  Provide “white glove” service in all interactions with patients, family, referral staff, etc. to positively impact the customer experience. Ensure consent for services is obtained and complete admission, if appropriate Develop and execute annual education plan in assigned facilities and with key stakeholders in partnership with marketing, sales, & clinical teams. Ensure a deep understanding of all services and knowledge of the referral process, features, and benefits to themselves and their patients/residents. Manage the organization’s transition program & discharge follow-up calls. Partner closely with clinical staff to ensure hospice readiness when appropriate. Work collaboratively to manage internal programs to transition hospice-appropriate home healthcare clients promptly and effectively.This includes making calls/visits to identified patients, when appropriate and managing discharge calendar for follow up Attend weekly ITD meetings to identify patient needsResponsible for identifying and resolving challenges and complaints to ensure client/partner facility satisfaction.Occasionally attend events within partner facilities to promote our services.The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
Education and Experience Required:
2+ years of healthcare experience as an RN within a home healthcare or hospice setting. Must have a current license. 2+ years of sales & customer service experience preferred. Must have adequate computer skills.Must complete and pass a state required background study.Able to physically perform the duties required by the position.Required to be able to read, write and speak English well enough to communicate with staff, patients, families, and facilities.
