The Perioperative Operating Room (OR) Registered Nurse (RN) is accountable for patient outcomes resulting from the nursing care for the patient undergoing a surgical/ invasive procedure. The perioperative OR RN plans, directs and coordinates the care of every patient undergoing operative and other invasive procedures, taking into consideration respect for the individual and recognition of the important contribution that each team member makes to the patient's well-being. This OR RN is expected to function proficiently in both the circulator and scrub role within the OR and to be competent in all surgical specialties served. He/she maintains knowledge of current Association of peri-Operative Registered Nurses (AORN) Standards, VHA Directives, and service level standard operating procedures. The incumbent assures the sterility of the surgical field is maintained and all instrumentation and equipment in play is used in a safe and efficient manner. The OR RN protects the patient from injury caused by chemical, electrical, mechanical, and thermal sources, and extraneous objects, performs counting procedures to protect the patient from unintended retained surgical items, participates in the Time Out process to ensure correct site, side, and level, and follow implant and tissue/tissue-based product documentation, policy, and procedures. The OR RN manages the patient's specimens in the appropriate manner, administers medications safely and correctly, performs interventions to maintain the patient's wound and tissue perfusion at or above baseline levels, and performs interventions to protect the patient from surgical site infection. He/she is responsible for ensuring comprehensive documentation of all care provided during the perioperative experience is timely, complete, and accurate in the electronic health record. The OR RN is required to be on-call during off tours hours to provide patient care for emergency procedures.
Duties include but not limited to the following:
- Responsible for providing nursing care to surgical patients. Includes the preoperative, intraoperative, and postoperative periods, along with expected behavior patterns and technical activities performed during the preoperative, intraoperative, and postoperative phases.
- Perioperative nursing practice is flexible and diverse and includes a variety of nursing roles that incorporate both behavioral and technical components of professional nursing. The scope of practice for each individual perioperative nurse may include, but is not limited to, roles as scrub person, circulatory, manager, and educator.
- Role of Circulator is vital to the provision of that care and includes but is not limited to:
- Responsibility for nursing activities related to the care of the patient in the operating room.
-Creation and maintenance of a safe and comfortable environment in which surgery can take place.
-Direct and indirect care of the patient which involves:
a. Use of nursing judgment and decision making.
b. Knowledge of an ability to use the nursing process in assessing, planning, implementing, and evaluating care.
c. Faculty for effective communication and the ability to recognize and intervene in stress situations involving the patient and/or other team members.
d. Implementation of the principles of medical and surgical asepsis.
e. Current knowledge of the legal implications of surgery.
f. Ability to act in an efficient, rational manner in emergencies.
g. Ability to interact skillfully with and supervise others.
-Acting as the patients advocate in meeting his/her physical and psychological needs.
- Activities of the role of the scrub person are considered an integral part of perioperative nursing practice, therefore the registered nurse who performs the role of the scrub person is practicing nursing.
- The registered nurse in the operating room acting in the capacity of either circulating or scrub nurse.
