SSM Health St. Mary s Hospital in St. Louis, Missouri, is seeking a dedicated and skilled Advanced Practice Provider (APP) to join our Primary Care Clinic team. The APP will provide comprehensive, patient-centered care, including assessment, diagnosis, treatment, and follow-up for a diverse patient population. This role involves collaborating with physicians, nurses, and other healthcare professionals to deliver high-quality care and improve patient outcomes.

Key Responsibilities:

Perform comprehensive health assessments, including patient histories, physical examinations, and diagnostic tests.

Diagnose and manage acute and chronic illnesses, injuries, and conditions.

Develop and implement individualized treatment plans in collaboration with patients and their families.

Prescribe medications and other treatments in accordance with state and federal regulations.

Educate patients and their families on health maintenance, disease prevention, and management of chronic conditions.

Perform routine procedures such as suturing, wound care, and immunizations.

Collaborate with physicians, specialists, and other healthcare professionals to ensure comprehensive patient care.

Document patient care activities accurately and timely in electronic health records (EHR).

Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines.

Stay current with medical advancements, evidence-based practices, and state and federal healthcare regulations.

Qualifications:

Master s or Doctoral degree in Nursing Studies from an accredited institution.

Current licensure as a Nurse Practitioner (NP) in the state of Missouri.

Certification as a Nurse Practitioner by a recognized certifying body (e.g., AANP, ANCC) or as a (NCCPA).

Minimum of 2 years of experience in primary care, family practice, or internal medicine preferred.

Strong clinical skills and the ability to work independently and as part of a multidisciplinary team.

Excellent communication, interpersonal, and organizational skills.

Commitment to providing compassionate, patient-centered care.

Proficiency with electronic health records (EHR) systems.
