For the RN Case Manager position described, the most important attributes and qualifications to look for in a candidate are:

Recent experience (within the last 12 months) working in acute care or home care settings as a case manager in the specialty area.Interdisciplinary Team Experience:Proven ability to work effectively with interdisciplinary teams, including doctors, physicians, social workers, dietitians, and health coaches.Strong Communication Skills:Exceptional communication skills for effective interaction with patients, families, healthcare providers, and interdisciplinary team members.Ability to convey care plans and collaborate with various stakeholders in the healthcare continuum.Patient-Centric Approach:Demonstrated commitment to optimizing patient care, addressing patients' health care needs, and supporting patients in achieving their goals of care.Experience with care coordination, discharge planning, and managing patients across different levels of care.Remote Work Capability:Ability to perform 90% of work remotely, including telephone services, with occasional on-site visits to clinics and in-person department meetings.Self-motivation and strong organizational skills to manage remote work efficiently.Knowledge of Care Coordination Tools and Systems:Familiarity with care coordination tools and electronic health record systems (e.g., Cerner) to process referrals, document care plans, and track patient progress.Flexibility and Adaptability:Ability to adapt to situational care coordination based on patient needs and payer requirements.Flexibility to manage episodic and longitudinal referrals and adjust the cadence of follow-up calls accordingly.Leadership and Problem-Solving Skills:Capacity to act in a leadership role for process improvement activities and collaborate on developing and managing interdisciplinary discharge plans.Strong problem-solving skills to identify and resolve barriers to effective patient care.Regulatory and Utilization Knowledge:Understanding of medical necessity, appropriateness of care, and regulatory requirements.Ability to collect and communicate pertinent information to payers and other regulatory bodies.Commitment to Continuous Learning and Improvement:Willingness to educate internal team members on case management and managed care concepts.Openness to training and development opportunities, such as cross-training in TCM.

Core Functions:

The RN Case Manager works to optimize the care of the patient in whatever way is needed so that the patients goals of care are completed and the patient is successful in their care of ADLs. This may include aligning home health, partnering with the HP for assistance with social determinants of health, or working with the care team to complete needed resources for care, including helping patients with decision around placement, respite care, etc.As the RN Case Manager, you will have the opportunity to do discharge planning for hospitalized patients to their next level of careManages individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomesActs in a leadership function with process improvement activities for populations of patients to achieve optimal clinical, financial, operational, and satisfaction outcomesActs in a leadership function to collaboratively develop and manage the interdisciplinary patient discharge planEffectively communicates the plan across the continuum of careEvaluates the medical necessity and appropriateness of care, optimizing patient outcomesAssesses patient admissions and continued stay utilizing standard criteriaIdentifies issues that may delay patient discharge and facilitates the resolution of these issuesEstablishes and promotes a collaborative relationship with physicians, payers, and other members of the healthcare teamCollects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirementsEducates internal members of the healthcare team on case management and managed care concepts
