Duties include but not limited to:
Independently manages a panel of patients with acute and chronic pain conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
Diagnose and treat opioid use disorder (OUD) and provide highly specialized, coordinated, comprehensive medication management service, to include but not limited to the prescribing or deprescribing of opioids for pain and the assessment and treatment of OUD.
Develops and implements an individualized plan to achieve optimal population health and disease management. This includes implementation of necessary risk mitigation strategies, and provides appropriate follow -up and monitoring as clinical indication warrants.
In addition to medication management of opioid therapy and OUD, and as aligned with the Stepped Care Model for Pain Management (SCM-PM) the NP will provide high-level focus and support to integrated pain care not only by the prescribing of appropriate non-opioid pharmacological alternatives but also, in coordination and collaboration with other clinicians within the Pain Resource Team (PRT) as applicable , by assessing for, providing, or assisting with appropriate referrals for nonpharmacologic treatment options as clinically indicated. The clinic focus is intended to provide a readily accessible treatment option for Veterans of a specialized PRT team-based care to ensure the highest quality service and outcomes to enhance Veteran's care experience.
Demonstrates leadership and works collaboratively with the interdisciplinary team.
Orders and interprets labs, tests, and diagnostic studies.
Prescribes evidence-based pharmacologic and non-pharmacologic interventions.
Initiates referrals and consults to specialty services.
