St. Luke's Health System in Boise, Idaho is seeking an Infectious Disease Clinical Pharmacy Specialist – Outpatient Antimicrobial Therapy

This clinic pharmacist will be implementing a new practice as an outpatient parenteral antimicrobial therapy (OPAT) pharmacist in collaboration with our infectious diseases and antimicrobial stewardship teams. Ambulatory pharmacy services provided will include addressing pharmacy-related questions from clinic providers and managing infectious diseases under collaborative practice.

This pharmacist is a member of the infectious disease (ID) clinic team and works closely with physicians and APPs to ensure appropriate long-term antimicrobials and monitoring.

Responsibilities

Assists with determination of OPAT candidacy and regimen selectionCounsels patients on OPAT regimensOversees pharmacotherapy monitoring for patients on OPAT serviceVerifies, orders, and reviews medications and labs for OPATManages arising symptoms or lab values and changes antimicrobials or follow up plan as neededDetermines if/which IV access is appropriateOrders IV access removal or otherwise verifies its removalProvides medication refillsEnsures formulary complianceSupports the System Antimicrobial Stewardship Team and Antimicrobial SubcommitteeCollaborates with System ID pharmacist, Antimicrobial Stewardship Team, physicians, and colleagues to ensure smooth hand off for transition to outpatient pharmacyCollaborates with Infectious Diseases/Antimicrobial Stewardship Program Coordinator on quality improvement initiativesSupports with transitions of care effortsOther duties as may be assigned

Qualifications

Degree from an accredited pharmacy program requiredNo experience requiredValid Idaho Pharmacist LicensePreferred qualificationsClinical management of primary care based infectious diseasesCompletion of PGY2 Infectious Diseases Residency OR equivalent experienceCurrent Board Certification in Infectious Diseases Pharmacotherapy (BCIDP)Why St. Luke's
