Hospitalist for Hospital-at-Home Program– San Antonio, Texas
University Health / University Medicine Associates is presently seeking a board-certified family or internal medicine physician with a passion for hospital medicine to provide care through our “Hospital-at-Home” (HAH) program. This unique program allows eligible patients who need acute/inpatient care to receive treatment at home instead of the hospital. This program has proven to reduce costs, improve outcomes and increase patient satisfaction.
Position details:
Work onsite at University Hospital in dedicated officeDay-shift – 7 days on / 7 days offFull-time advanced practice provider supportConduct the History & Physical Exam on patients transitioning to the HAH program when admitted from the emergency room or meet with patients currently inpatient before transferring to HAHConduct one telemedicine visit with the HAH patients per day (Any needed home visits are conducted by a registered nurse or community medical team)Complete discharge summaries on patients discharged after completing stay from HAHRounding in hospital to assess patients who may qualify for the programWhen working must be available for after-hours phone calls (minimal), if the patient needs to be admitted back to the hospital, the patient will transferred back to the inpatient hospitalist serviceThe expectation is 18 tele-visits and rounds on the floor for transfers to HAHWhat we offer:
