Clinical Pharmacy Specialist, Day Shift, Psychiatry

As a Clinical Pharmacy Specialist in Psychiatry you will:
• Conduct medication group and discharge counseling
• Assist with admission and discharge medication reconciliation and streamline/ensure appropriate formulary use
• Improve medication adherence, reducing antipsychotic polypharmacy, and providing a patient-centered care through shared decision making
• Monitor for drug-related adverse events, reduce the overuse of psychotropic medications and provides comprehensive medication management through collaboration with the care team
Qualified candidates will possess:
• Graduate from an ACPE accredited pharmacy school, with PharmD degree required
• ASHP accredited PGY-2 Residency, or PGY-1 residency with Minimum of 3 years of clinical pharmacy experience in area of specialty required
• Registered Pharmacist with the Maryland Board of Pharmacy
Preferred (required within one year of employment): Board Certification in area of specialty required
• ACLS Certification preferred
• Member of American Society of Health-system Pharmacists (ASHP), and/or Maryland Society of Health-system Pharmacist (MSHP), or other relevant professional organization preferred
• Prior experience precepting students and residents preferred

Tobacco and Drug Statement
Tobacco use is a well-recognized preventable cause of death in the United States and an important public health issue. ("Nicotine products" include, but are not limited to: cigarettes, cigars, pipes, chewing tobacco, e-cigarettes, vaping products, hookah, and nicotine replacement products (e.g., nicotine gum, nicotine patches, nicotine lozenges, etc.).
