**Conviva Senior Primary Care** in Palm Beach is looking to add a new Primary Care Physician to our team in **Delray Beach, Florida**.
_**About the role:**_
- **Monday-Friday, 8-5 Hours** in an Outpatient Setting
- See **12-15 patients per day** under a value-based care model
- Team-based model with daily huddles
- Full support staff including care coaches, MAs and APPs
- eCW EMR system
_**Benefits:**_
- Base salary plus 20% quality incentive pay, for total earnings up to $300,000
- Sign-on bonus, relocation offered in some circumstances
- Occurrence-based malpractice
- Full benefits including 401k matching, CME time + funds, health and life insurance, etc.

_**Requirements:**_
- BC/BE
- Florida Licensure
